HEALTH GUIDELINES ACKNOWLEDGEMENT
Please keep these guidelines in a safe place and refer to them if your child becomes ill.

Do not send your child to school if your child has any of the following:
• A temperature of 100 F/37.8 C in the last 24 hours
• Vomiting or diarrhea in the last 24 hours
• Redness and/or discharge from eyes, crusting in lashes (conjunctivitis)
• Contagious skin rashes or uncovered draining wounds
• Strep throat (can return to school 24 hours after antibiotic treatment begins)
• Influenza/viral infection (requires medical clearance with a return to school date)
• Contagious disease such as chickenpox, mononucleosis, conjunctivitis
(requires medical clearance with a return to school date)
• Emergency Room visit (return to school with a copy of the ER discharge form)
• COVID-19 symptoms:
Fever of 100.4 or more, chills, cough, shortness of breath/difficulty
breathing, fatigue, muscle/body aches, headache, new loss of taste or smell,
sore throat, congestion or runny nose, nausea/vomiting/diarrhea
If your child develops symptoms of COVID-19 during the school day, he/she will be isolated, and you will be called to pick up your child. Your child may return to school with medical clearance. Please refer to Quarantine /Isolation Guidelines in Accordance with Contact Tracing and the Required Return to School Medical Clearance forms.
